1. A copy of your “Individual number (My Number) card” (including photograph), 2. a copy of your “Notification card,” and 3. the original “Certified copy of resident register” or original “Certificate of items entered in the resident register” verify your ID. A notification explaining the required documents will be sent to your registered e-mail address on or after the business day following your application.
